Through the Oklahoma Department of Human Services, Developmental Disabilities Services Division’s Community Waiver Program. The Community Waiver Program includes a comprehensive array of services including residential, employment and habilitation services and support – appropriate to the individual’s needs. A plan of care is developed annually for each individual receiving services through the Community Waiver. Using a person-centered planning approach, the individual’s team of interdisciplinary professionals assess the needs of the individual and develops a plan to address those needs.
